Newly born

The affiliation for newborns is done by presenting only the following documents:

  • If there is no marriage, Affidavit of Coexistence. This document is legalized in the Civic Registry with two witnesses.

  • If it is the case, Death Certificate (photocopy).
  • Marriage certificate with canceled departure and divorce decree, if given.
  • Four (04) Forms DS-07. These correspond to the Request for Registration of Beneficiaries. Must be filled out correctly and printed on legal size sheet. Do not forget the signature and the seal.
  • Certification Form of the Health Insurance, that there is no short-term affiliation. You must fill it out correctly and print it on a legal-size sheet, mandatory.

  • Social Work Report.
  • SIN Certificate (National Tax Service), without NIT.
  • Finally, three (03) photos, which must have the following characteristics:

Siblings

  • Birth certificate. You must bring both the original computerized and the photocopy for a correct registration of the data. The latter must be legible, in black and white.
  • Parents’ Marriage Certificate. Again, you need both the original computerized and the photocopy for the record. The latter must be legible, in black and white if you like.
  • Identity card. This must be current and you must attach both the original and the photocopy. Again, in the case of the latter, it must be legible.
  • If it is the case, Death Certificate (photocopy) or divorce decree of both parents.
  • Four (04) Forms DS-07. These correspond to the Request for Registration of Beneficiaries. They must be filled out correctly and printed on legal size sheet. Do not forget the signature and the seal.
  • Certification Form of the Health Insurance, that there is no short-term affiliation. You must fill it out correctly and print it on a legal-size sheet, mandatory.

  • SIN Certificate (National Tax Service), without NIT. Only if both parents are alive.
