  6. Remove your password from the email you have provided.

The recommendation is that the responsible adult be the one who fills out the form, as any error may affect the future issuance of the document. Likewise, if the form is filled out by the minor, the guardian should review it carefully to make sure everything is okay.

Regarding payment, this fee must be paid through a savings or checking account that is enabled for it.
