How is the care center assigned?

The Assistance Center is designated by EsSalud for the medical care of the Insured by ID number.

This number shows the corresponding information and the nearest center will be assigned to it.

EXAMPLE: If you reside in the district of Breña, the Chincha Polyclinic will be assigned as a place to be treated.

Requirements for change of Center

However, if you are assigned a center that is relatively far away or for some reason, you prefer to attend another, you must do the following:

That is, surnames and names, type and number of identity document, date of birth.
